indicated that there are less than five other active gas utility
accounts not in your name located at your building.
To process your
request, you must provide SoCalGas with signed authorization to
receive customer information or act on a customer's behalf. The
account holder(s) must sign the Customer Information Standardized
Request (CISR) Form.

form(s), please upload the completed form(s) on Step 3. The CISR
form(s) must be completed before moving to the next step.
